Background:
The State of Florida has been granted waivers of certain Title XIX requirements to provide special Medicaid services to specified groups of people. The Assisted Living Medicaid
Waiver provides additional services to allow persons who would otherwise need nursing home care to remain in an Extended Congregate Care (ECC) or Limited Nursing Services (LNS)
Assisted Living Facility (ALF).
Who Should Be Referred:
Persons who are 60 years of age or older and eligible for on-going Medicaid under SSI, MEDS-AD or ICP. They should also be functionally impaired, to the extent that services beyond
those basic services provided by an assisted living facility are needed to allow the senior to reside in an ALF and prevent nursing home placement.
Services Provided Include:
Case Management - Each client is assigned a case manager to assess the additional services required to ensure that the elderly individual can reside in an assisted
living facility.
Support Services - Assistance with bathing, behavior management, companionship, medication management, medical supplies, nursing services, additional housekeeping, therapies
and other activities which may be therapeutic or supervisory in nature.
Funding:
Funding is made available through the West Central Florida Area Agency on Aging, State of Florida Department of Elder Affairs, State of Florida Medicaid Program and the
Hillsborough County Board of County Commissioners.
