On September 21, 2011, the Hillsborough County Board of County Commissioners approved an ordinance to address the growing epidemic of fraudulent insurance claims that have been linked to clinics operating in Hillsborough County, which primarily treat patients allegedly injured in staged auto accidents.
*Note to PIP Clinic License applicants: Fingerprint cards can be obtained at the Consumer Protection Agency located at 1101 E 139th Ave Tampa, FL 33613, or the Florida Department of Law Enforcement Tampa Bay Regional Operations Center located at 4211 N Lois Ave Tampa, FL 33614. Each fingerprint card must include the applicant's full name, sex, race, and date of birth; the subject's social security number is optional.
