Head Start/Early Head Start Participation Application
To place a child or expected child in the Hillsborough County Head Start/Early Head Start program, an application must be completed. Please click on one of the links below, for either an English or a Spanish version of the application, print it out, and fill it in completely. Applications are also available at any Head Start/Early Head Start Center, Neighborhood Service Center, Food Stamp Office, TANF site, and the Head Start/Early Head Start Central Office (3639 W. Waters Avenue, Suite 500, Tampa, FL 33614) or by calling (813) 272-5140.
