Hillsborough County Health and Social Services would like to welcome you to the Hillsborough County Health Care Plan (HCHCP). This website is for HCHCP network participating providers. The provider website provides information on member eligibility, medical services covered by the plan, formulary information, referral and authorization requests, billing inquiries and information, a provider newsletter and useful hotlinks to other related clinical and educational sites.

This website also allows participating providers to enroll prospective members on-line to facilitate patient care. The website also contains reference materials such as the provider guide and a quick reference formulary wall-chart for providers. The website also provides information on the mandatory Patient Assistance Program (PAP) for medications that qualify for the PAP Program.
