To apply for Hillsborough HealthCare, you must complete the following steps:

	Step 1 - Complete the Application Worksheet Each adult must sign Page 3 of the Application

	Step 2 – Read and Sign the Forms (Print one set for each adult ) Each adult must sign one set, on Page 2 and Page 3) Review On-line or Print the Hillsborough County Notice of Privacy Practices so you can acknowledge you read it when you sign your forms

	Step 3 - Gather required documentation to prove your eligibility Read "How to Apply"

	Step 4 - Bring all of these to an enrollment site for processing Read "Where to Apply"
