- DD 214 (copy 4) Required to submit claim when you click on DD-214)
- VBA-21-2680 ( PDF, 213KB ) (Examination for Housebound Status or Permanent Need for Aid & Attendance)
- To apply, bring DD 214 (copy 4), 21-526, 21-22 to one of our locations. A County Veteran Service Officer will assist in completing the application process and advise you of future actions required.
- You may be required to provide additional documents such as a marriage certificate, death certificate and financial information.
