Things to consider
- To best facilitate scheduling, please submit presentation requests at least six weeks in advance
- We may not be able to accommodate all requests due to staffing availability

Fill out the form and provide the following information:
- Requestor name
- Requestor daytime contact number
- Requestor email
- Group or organization name
- Requested presentation location
- Requested presentation date
- Requested presentation time
- Alternative date(s)
- Number of expected attendees
- Preferred presentation topic(s), such as:
- Preventing mosquitoes at home
- Preventing mosquitoes in the workplace
- Mosquito management techniques
- The lifecycle and habitats of common mosquitoes
