Hillsborough County is administering free vaccinations with an appointment. County sites currently offer primarily Pfizer vaccinations, with some sites offering the Moderna vaccine on a limited basis. There will be no vaccines for persons without appointments.
Please Note: Operations vary weekly depending on vaccine availability. Sites may not offer first doses every week.

Appointments for vaccinations at a County-operated site are mandatory. There will be no vaccines for persons without appointments.

- Create an account with CDR Health Pro by visiting patientportalfl.com. You cannot receive an appointment without first creating an account.
- Select from available appointments for your first dose and your second dose appointment will be automatically assigned.
*16- and 17-Year-Old Teenagers and Pfizer Vaccine
- The Pfizer vaccine is available for 16- and 17-year-old teenagers.
- The parents and legal guardians of teenagers, who are 16 and 17 years old, must be present at the time the minor receives the Pfizer vaccine.
- An online consent form must be completed when a parent or legal guardian makes the appointment for the teenager to receive the vaccine at a Hillsborough County site. Without a completed consent form, the vaccine will not be administered to the minor.
- The Pfizer vaccine is the only vaccine that can be administered to 16- and 17-year-old teenagers. The Moderna and Johnson & Johnson vaccines are for ages 18 and over.
Making An Appointment
Is there a cost to be vaccinated?
There is no out-of-pocket cost to be vaccinated. A doctor’s note or prescription is not required. Health insurance information is being collected in accordance with federal guidance; however, you will not be asked to pay a copay or deductible. Individuals who do not have health insurance will still be vaccinated.
What health care workers are being vaccinated?
Health care workers qualified for a vaccine by Executive Order of the Florida Governor are those individuals with direct patient contact. Examples include physicians, paramedics, emergency medical technicians, phlebotomists, nurses, home health providers, home medical equipment providers, therapists, dental office staff, pharmacists, and administrative/reception staff among other health care roles (including law enforcement officer, EMTs, fire fighters, school nurses).
Will I be able to make an appointment for my spouse/partner to receive the vaccine at the same time?

What vaccine is available at the County’s public vaccination sites?
At this time, the Pfizer-BioNTech COVID-19 vaccine is being dispensed.
How many doses of the vaccine are required?
According to the Centers for Disease Control and Prevention, two doses are the most effective.
When are second-dose appointments made?
When you select an appointment for the first dose, the appointment system will automatically assign a date and time for your second dose.

What happens if I did not receive an appointment confirmation?
It is very important to enter accurate and complete information into the registration portal. If you do not receive the confirmation QR code (via email, and text if applicable), call the CDR Maguire helpline at (850) 848-5287 or email Covid19support@cdrmaguire.com.
Can I arrive early for my appointment?
No. Please do not arrive ahead of your scheduled appointment time. Early arrivals create traffic congestion and line delays. Those arriving early will be turned away and instructed to return at their appointment time.
Do you need a ride to get a COVID-19 vaccine?
Do you have Medicaid? If yes, Florida Medicaid will take you to get the COVID-19 vaccine at no cost. See more information about Medicaid transportation for COVID-19 vaccinations or call the Medicaid Helpline at 877-254-1055.

- Printout or digital screenshot of appointment QR code
- If applicable, proper work credentials
- If this is your second dose, also bring your Vaccine Record card that you received on your first visit.
- The Centers for Disease Control and Prevention recommends that people wear a mask that covers their nose and mouth when receiving any vaccine, including a COVID-19 vaccine. Anyone who has trouble breathing or is unable to remove a mask without assistance should not wear a mask.
What proof of residency items should I bring?
The Florida Division of Emergency Management asks that all individuals bring proof of Florida residency to their vaccine appointment. One document is sufficient. Examples of proof of residency include:
- A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
- One proof of residential address from the seasonal resident's parent, stepparent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
- A utility hookup or work order
- A utility bill, not more than 2 months old
- Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
- Mail from a federal, state, county, or municipal government agency, not more than 2 months old.
What can I expect when I arrive at the vaccination site?
- Only people with an appointment will receive a vaccination. This includes second doses. People who arrive without a reservation, including other people accompanying you, cannot be vaccinated.
- At drive-thru locations, you must stay in your vehicle throughout the entire process, including administration of the shot. If you arrived to a drive-thru location via a taxi or ridesharing service (such as Uber or Lyft), you are required to stay in that vehicle.
- At indoor locations, you will park your vehicle and proceed inside. Check-in, vaccination, and observation will all take place inside the building.
- You will be greeted by staff who will verify your appointment and check you in. They will ask a series of questions to assess your situation and level of comfort about receiving the vaccine.
- The next stop is the clinical area, where the vaccine will be administered. Trained medical personnel will administer the vaccine. The injection will be given in the upper arm; wear clothing that can easily accommodate this.
- You will be given information about symptoms to watch for in the event of an adverse reaction to the vaccine and instructions on what to do. You will also receive a vaccination card. Keep the card in a safe place and ready for your second dose.
- You will be directed to a waiting area for an observation period that will last approximately 15 to 30 minutes.
Will I receive a separate confirmation for the second dose?
You will receive the second-dose confirmation at the same time as you receive the confirmation for the first appointment. You also will receive a reminder for the second dose.
When will I receive a COVID-19 Vaccine Record Card?
All people who receive a COVID-19 vaccine will be provided a Vaccine Record Card at the time of the first injection. This card will include important information such as patient name, type of vaccine received, date of first dose, date of second dose, and the location the vaccines were administered. You must bring the Vaccine Record Card with you when you return for the second dose. You also are encouraged to keep the card in a safe place for future reference.
What can I do if I lost my COVID-19 Vaccine Record Card?
If you lose your vaccination card or any record of it, return to the location that provided the vaccination if possible. If you do not have success at the location or if the location is no longer in operation, and you received the vaccine in Hillsborough County, contact the Florida Department of Health in Hillsborough County or call (813) 307-8000 and select "immunizations" option 3. Please be aware that it may take a while to research and process the request.
Your primary care physician's office also should be able to look up the record of your immunization.
What should I do if I have a reaction to the vaccine when I get home?
Follow the instructions provided during your visit. The Centers for Disease Control also provides information regarding reactions. If you think you might be having a severe allergic reaction after leaving the vaccination site, seek immediate medical care by calling 911.
Why aren’t there more appointments available?
Quantities of the vaccine are limited, and demand is overwhelming. Currently, the number of appointments is based on the number of doses available. As availability of the vaccine grows and demand evens out, it will be easier to ensure everyone who qualifies receives an appointment.
